Disaster Assistance

KY Supplemental Nutrition Assistance Program (SNAP) recipients: Households that lost food purchased with their SNAP benefits due to the February 15th, 2025 storms and flooding have until Wednesday, February 25th, 2025 to request replacement benefits.
Recipients who lost food bought with SNAP due to a power outage of at least four hours or flooding can call the KY Department for Community Based Services (DCBS) at 855-306-8959 to request replacement benefits. If possible, please have your case number ready.

 

Passport by Molina Healthcare is here to support you. We will post information with regard to emergency and/or disaster assistance as needed in this location.

  • Passport Member Services Line: 800-578-0603 – we can help with replacing lost member ID cards, assist in obtaining replacement durable medical equipment, and more

     

  • Behavioral Health Crisis Line: 844-800-5154 – licensed clinicians are available to assess members in crisis, provide crisis counseling and connection to services

     

  • Care Management Referrals: Providers can call 800-578-0775, select the Care Management option. Or anyone can email CareManagement_KY@passporthealthplan.com - this includes referrals for housing assistance and assistance with other social determinants of health (e.g., food, clothing, transportation)
