Housing Services

Rent assistance and support

If you need help finding a place to live or help paying for housing, Molina can help connect you to community and housing resources in your area.

If needed, Molina will refer you to the Regional Behavioral Health Authority to Agreement (RBHA) for Non-Title XIX/XXI services and local community housing providers. Please call Member Services at (800) 424-5891 (TTY/TDD: 711) Monday-Friday 8 a.m. to 6 p.m. MST for help. We have a care management team who can help you get set up with these services.

Getting help from housing specialists

Our housing specialists can help if you are homeless or at risk of losing your home. We can:

  • Give you resources within your area to address eviction prevention, potential rental assistance resources, utilities assistance resources, and short-term living opportunities.
  • Make any referrals to contracted housing providers for individual assistance in locating housing opportunities, navigating evictions, or renewal of housing program paperwork including the AHCCCS Housing Program (if eligible), if needed.
  • Help you find options that accept rental subsidy or vouchers, if needed this can also include a referral for housing navigation services through a contracted housing navigation provider.
  • Help you find short term shelters through Coordinated Entry providers, if needed.


We work with local agencies to find funding and ways to support those individuals that are experiencing homelessness within their communities. We also train our staff and community agencies on housing needs, resources and their impact.

The AHCCCS Housing Program is a rental subsidy and eviction prevention program that assists members that qualify with housing assistance. While housing is not a covered service, there are limited funds to assist with eviction prevention, utility assistance, and rental assistance for those members that qualify. AHCCCS Housing Programs (AHP)

In order to participate in the AHCCCS Housing Program (AHP), members of Molina Healthcare who have been identified as having a housing need and fit eligibility requirements are able to work with their clinical team to apply for a housing subsidy, eviction prevention funds, move-in assistance funds, and utility assistance. Health and Housing Opportunities (H20) Program

The AHCCCS H2O demonstration aims to improve and grow housing services and interventions for members who are homeless or in danger of homelessness.

H2O Eligibility

To use the H2O services covered by this waiver, you must be eligible under Title XIX and meet additional H2O eligibility criteria described here:

  1. Eligible members must be Homeless and 18 years of age or older. In accordance with 24 CFR 91.5, the U.S. Department of Housing and Urban Development (HUD), beneficiaries must be homeless or at risk of becoming so.
  2. Eligible members must also have an SMI designation and be 18 years of age or older. Recipients must have a chronic illness and a Serious Mental Illness Designation.
    1. Recipients who have been released from a correctional facility within the last 90 days, or who have been designated as having a Serious Mental Illness (SMI) and are presently detained in a correctional health facility with a scheduled release date within 90 days.
    2. When determining eligibility, a person's SMI and/or move out of an institutional setting take precedence over ongoing medical issues.
    3. Chronic Health Conditions included are:
      1. Hypertension
      2. Heart Disease
      3. Heart Failure
      4. Liver Disease
      5. Diabetes
      6. Renal Failure
      7. Kidney Disease
      8. Kidney Transplant Failure
      9. Necrotizing Fasciitis
      10. Liver Cancer
      11. Skin Cancer
      12. Multiple Sclerosis

While this program is not currently eligible for all AHCCCS Complete Care (ACC) participants enrolled within Molina at this time, for those that are going through the Serious Mental Illness (SMI) Designation process and are approved, this program may be a benefit after that transition to SMI Services.
