• Senior Whole Health (HMO D-SNP)     

  Senior Whole Health (HMO D-SNP) is a Medicare Advantage Special Needs plan for seniors aged 65 and over that combines your Medicare, MassHealth Standard (Medicaid) and Medicare Part D benefits in one convenient plan.

  You are eligible to join the Senior Whole Health (HMO D-SNP) plan if:

  • You reside in the geographic service area
  • – and – you must have both Medicare Part A and Medicare Part B
  • – and – you are eligible for MassHealth Standard (Medicaid)
• Senior Whole Health NHC (HMO D-SNP)     

  Senior Whole Health NHC (HMO D-SNP) is a Medicare Advantage plan for seniors aged 65 and over who are eligible for both Medicare and Medicaid benefits.  Individuals requiring nursing home level of care receive home and community-based support services that allow them to remain comfortably in their own homes.

  You are eligible to join the Senior Whole Health NHC (HMO D-SNP) plan if:

  • You reside in the geographic service area
  • – and – you must have both Medicare Part A and Medicare Part B
  • – and – you are eligible for MassHealth Standard (Medicaid)
• Senior Whole Health Senior Care Options (SCO)     

  Senior Whole Health SCO plan offers Medicaid, Medicare and Part D benefits to seniors with MassHealth.

  You may qualify if:

  • You are 65 years or older
  • You have MassHealth (Medicaid)
  • You live in our service area
• In certain situations, you will be denied enrollment in Senior Whole Health     

  Massachusetts seniors will be denied enrollment to Senior Whole Health Senior Care Options (SCO) plan, Senior Whole Health (HMO D-SNP) or Senior Whole Health NHC (HMO D-SNP) if:

  • – You do not meet the eligibility criteria listed above
  • – You are subject to a six (6)-month deductible period (spend-down)
  • – You are a resident of an intermediate care facility for the intellectually disabled
  • – You are a patient in a chronic disease or rehabilitation hospital
