Are you eligible for Molina Healthcare HealthChoice Illinois?

You may be eligible if: you are an older adult and/or an adult with a disability and eligible for Medicaid but not Medicare, you or your child is eligible for full Medicaid benefits.
To enroll with Molina Healthcare HealthChoice Illinois, you also need to live in our service area.
If you are eligible, you would have received information from Illinois Client Enrollment Services.
