Health Homes
Molina Healthcare understands that our members’ health is impacted by many different factors and that coordinated care between behavioral health and physical health providers, also known as integrated care, is important to our members. One of the ways we provide integrated care is through health homes.
What is a health home?
A health home is a healthcare provider that provides or coordinates all of your behavioral health and physical health services including:
- Medication services
- Case Management
- Transportation
- Peer and family support services
- Individual and group therapy
- Health and wellness groups
- Follow up after receiving crisis services or going to the hospital
Why was I assigned to a health home?
Members are assigned when Molina believes they will benefit from a health home, including our members with a Serious Emotional Disturbance Determination.
Do I have to go to my health home for services?
No. Even though you have a health home, you have the ability to choose where you want to receive services from
How do I change my health home assignment?
Please call Member Services to request a change to your health home assignment.
Molina Healthcare has developed partnerships with specific providers to serve as health homes for our members in order to ensure high quality services. While other providers might meet the health home definition, members can only be assigned to members that Molina has specifically identified as a health home as listed here.
