2024 Provider Satisfaction Survey

How to complete the 2024 Provider Satisfaction Survey:
- Randomly-selected providers should have soon receive a packet in the mail from Press Ganey, an NCQA-certified survey vendor, on behalf of Molina.
- You may complete this packet and return it in the mail, or scan the QR code in that packet to complete the survey online at www.sphsurvey.com.
- If you'd like a list of providers from your practice who received the survey, reach out to MHWIProviderNetworkManagement@MolinaHealthcare.com.
Didn’t get the survey?Please call SPHA at 1-800-588-1659.
Have multiple locations? You can respond to ONE survey and have the responses applied to multiple surveys. Be sure to include this information in your survey response.
