Community Health Assessment and Community Health Improvement Plan (CHA/CHIP)
Molina collaborates with each Local Health Jurisdiction (LHJ) and other managed care organizations in each of our counties we operate in to support the Community Health Assessment and Community Health Improvement Plan (CHA/CHIP) process. By means of sharing data, providing funding and resources, participating in steering committees, and engaging members and providers on CHA/CHIP activities, Molina aims to improve the health and wellness of our communities.

Sacramento
The Sacramento County Community Health Improvement Plan (CHIP) is a 5-year action plan aimed to coordinate efforts and target resources to address root causes of health inequities in Sacramento County. The plan was developed and will be implemented by a CHIP coalition consisting of local organizations, individuals, and County representatives.
San Bernardino
Community Vital Signs (Vital Signs) is a community health improvement framework jointly developed by San Bernardino County residents, organizations, and government. It builds upon the Countywide Vision by setting evidence-based goals and priorities for action that encompass policy, education, environment, and systems change in addition to quality, affordable and accessible health care and prevention services. It provides the basis for aligning and leveraging resources and efforts by diverse agencies, organizations, and institutions to empower the community to make healthy choices.
