Integrated Delivery System
Integrating the delivery of behavioral and physical health care is essential in improving the overall health of members. From a member perspective, this approach improves individual health outcomes, enhances care coordination, and increases member satisfaction. From a system perspective, it increases efficiency, reduces administrative burden, and fosters transparency and accountability.
Coordinating and integrating care is expected to produce improved access to primary care services, increased prevention, early identification, and intervention to reduce the incidence of serious physical illnesses, including chronic disease. Increasing and promoting the availability of integrated, whole person care for members with chronic behavioral and physical health conditions will help members achieve better overall health and an improved quality of life.
Molina recognizes the value to optimizing integration and we consider the behavioral health and physical health care needs of members in our network design and clinical operations.
Health Homes
One of the ways we provide integrated care is through having healthcare providers called health homes within our provider network.
What is a health home?
A health home provides or coordinates the provision of all primary, physical health, behavioral health, and services and supports to treat the whole person.
They provide, or coordinate, a range of recovery-focused services to members such as:
- Medication services
- Case Management
- Transportation
- Peer and family support services
- Individual and group therapy
- Health and wellness groups
- Ensure follow up after the member has received crisis services or discharged from an inpatient setting
Why is a member assigned to a health home?
A member is assigned to a health home when it is identified that they might benefit from this type of service including but not limited to those who have a Serious Emotional Disturbance designation and who score high on a level of care behavioral health assessment.
Does a member have to go to their health home?
No, the member is not required to receive services from their health home. Regardless of assignment, the member retains their ability to choose where they receive services from and can continue to receive services from any behavioral health or physical health provider they would like.
How does a member request a change to their health home assignment?
A member can request a change to their health home assignment by calling Member Services.
Molina Healthcare has developed partnerships with specific providers to serve as health homes for our members in order to ensure high quality services. While other providers might meet the health home definition, members can only be assigned to members that Molina has specifically identified as a health home in the list below.
