2025 Molina Member Rewards Program!
Getting your Molina rewards* just got easier! You can now submit your information through your member portal at MyMolina.com.

Molina's new reloadable reward card lets you continue earning rewards for completing important wellness visits and screenings. Shop in person at participating retailers or online at Walmart.com. You may also use your rewards for other purposes such as bill pay, gas (pat-at-pump only) or pet supplies. For details on where you can spend your rewards, see our Store Finder flyer.

Claiming your rewards is easy!

  1. Complete one or more of the health screening(s) mentioned above.
  2. Log in to your member portal at MyMolina.com.
    3. Note: Rewards can be submitted either under an individual's My Molina® member portal account or under the primary insurer's account. If you do not have an account, you will need to create one or call Molina Member Services at (800) 869-7165, TTY: 711 for help claiming your reward further.
  3. Click on the 'My Wellness & Healthy Rewards' tab, then click the 'My Healthy Rewards' tab.
  4. Click on the 'Request Submission' button to view the secure online form.
  5. Fill out the form with details of the health screening(s) you have received. You can submit multiple screenings at the same time.
  6. Click the 'Submit' button, and you're done!

 

Reward Delivery:

You will receive your reloadable reward card in the mail with additional information in about two to four weeks. If you have already received your card, it will be reloaded with the reward amount in one to two weeks.

Important Information:

  • To earn a reward(s), you must have Molina Healthcare of Washington as your primary insurance when service is given.
  • Apple Health members can get up to $200 in total rewards every 12 months.
  • Each reward can be received no more than once a year. For example, a diabetes HbA1c test can be completed every 3-6 months, but you can receive only one $35 reward each year.
  • All reward submissions for services done in 2025 must be submitted by 1/31/2026. Submissions received after the deadline will not be processed.
     

 

LIMITED TIME MEMBER REWARD!
Get your flu vaccine (shot) and earn a $50 Reward! You must receive your flu vaccine between September 1, 2024, and March 31, 2025 to be eligible for this reward. This offer is available to Apple Health (Medicaid) members ages 6 months and older.

This reward is in addition to your $200 yearly reward maximum.
Talk to your doctor about the flu vaccination.

After you or your child get your flu shot, let us know by providing your details using the member rewards submission process mentioned below. It's that easy! 

Note: Molina Apple Health members ages 6 months and older are eligible for this reward. To earn the reward, you must have Molina Healthcare of Washington as your primary insurance at the time of receiving your flu vaccine. You have until April 30, 2025, to submit your vaccine details.
