Molina Healthcare’s Healthy Actions Rewards Program was made just for our members. We want to support you on your health journey! Earn rewards for completing your healthy actions.

What Are Healthy Actions?

Healthy actions are routine screenings you complete with your doctor. These visits are important to keep you healthy. They give your doctor time to come up with a care plan for you. These visits can also catch health problems early.

Follow these steps to earn your rewards:

  1. Complete your healthy actions with your doctor between 1/1/2025 and 12/31/2025.
  2. Let us know you saw your doctor and what actions you completed by 1/31/2026.
  3. Claim your rewards by 1/31/2026.
  4. Use your rewards by 3/31/2026.

Complete these Healthy Actions to earn your rewards:

SCREENING

DESCRIPTION

Annual Wellness Visit

Yearly in-person check up with your doctor or in-home visit with Molina’s Care Connections Team

Colon Cancer Screening:

Screening to check for signs of colon cancer in adults ages 45-75. Check with your doctor to see which screening is best for you!

Breast Cancer Screening:

Mammogram to check for signs of breast cancer in women ages 50-74

Diabetes Screenings

Yearly diabetes screenings for adults ages 18-85 with diabetes. *You must complete all 3 screenings to claim your reward.

  1. A1c Test: Blood test to check your blood sugar levels
  2. Diabetic Eye Exam: Yearly eye exam with your eye doctor to check your eyes for signs of damage caused by diabetes
  3. Diabetic Kidney Health Test: A urine and blood test to check your kidney health

 

Flu Shot

Yearly vaccine to help lower your chances of getting the flu

You must be an active Molina member to claim your rewards. You will lose access to unspent earned rewards upon disenrollment with Molina.

Want to complete some screenings from the comfort of your home? Molina can mail you a home lab kit. The lab kits can check:

  • Your A1c level
  • Your kidney health
  • For signs of colon cancer

How Do I Claim My Rewards?

You can claim your rewards by:

  1. Phone

    Call the Wellness Team. Let them know:

    • When you saw your doctor
    • What healthy actions you completed

     

  2. Mail

    Print and fill out the form linked here. Mail the form back to Molina in an envelope with a stamp to:

  3. Nations Benefits Portal

Visit molina.nationsbenefits.com and enter your appointment information after your visit.

 

How Will I Receive My Rewards?

Your rewards will be loaded onto your Benefits Mastercard Prepaid Card.

 

Where Can I Use My Rewards?

You can use your rewards to buy healthy foods and produce. You can use this card at participating retailers. You can visit molina.nationsbenefits.com for a full list of retailers and eligible items.
