One of the best ways to stay healthy is by going to the doctor once a year. This visit helps your doctor know how you are doing and gives you the care you need, even if you feel fine. The good news is that this visit is covered with Molina.

If you go to your annual wellness visit and answer some questions about your health you can earn a $200 gift card!*

What is Molina Healthy Rewards?

Molina Healthy Rewards is a platform with programming to improve your health. The platform hosts free tools on topics like healthy eating, managing your cholesterol, how to quit smoking and more. You can get a gift card by answering some questions about your health and visiting your doctor for your yearly check-up.

How to get your gift card:

Step 1: Log into your My Molina account and click on the My Wellness page. Then, find the Molina Healthy Rewards tab.

Step 2: Click Assessments and then Health Risk Assessment to answer a few questions about your health.

Step 3: Make an appointment and go to your yearly doctor visit. You can find a doctor near you using our search tool or call (833) 982-1452 (TTY: 711), Monday - Friday, from 8 a.m. to 6 p.m. local time.

Step 4: After your annual wellness visit and health questions are completed, we will email you steps on how to get your gift card. Please allow 6-8 weeks after we receive your claim for gift card processing.

*Gift cards available to qualifying members. Molina Healthy Rewards is a voluntary program. It is available to all subscribers and dependents 18 years or older at no cost.
